Course Description and Statement of Need

Acute and chronic medical conditions during childhood frequently require care by pediatric specialists. However, due to changes in the healthcare delivery environment, there is growing awareness of the value of the medical home model. The pediatric primary care provider will be responsible for coordinating care along with referrals for specialty care and other health care and educational services. This will put the responsibility on primary care providers to implement evidence based best practices in a variety of pediatric medical conditions. This course is designed to address that gap, by providing information about the role of tertiary specialty care, with a special emphasis on multidisciplinary team approach regarding common, yet challenging, pediatric conditions.

This symposium will provide pediatric providers with the latest information regarding the evaluation, treatment and management of complex conditions commonly seen in the pediatric medical home. Participants will gain an understanding of the state-of-the-art in evaluation and treatment of these conditions. Expert faculty will provide participants with an extensive review and the necessary tools and treatment strategies to successfully confront these conditions in the medical home.

The symposium is designed to introduce real-life solutions that are currently applicable and available to the primary care practitionaire. The course will include both didactic and case-based sessions, with real-time audience polling. Audience participation and interaction will be strongly encouraged. Participants are requested to bring interesting or difficult cases for faculty panel discussions.

Participants of this program will be provided with additional resources to enhance and sustain their learning, including electronic versions of the course material and web links to access more information regarding the subjects discussed at the conference.

Target Audience
This educational activity is intended for general pediatricians, family practice clinicians and pediatric nurse practitioners in academia and the community.

Educational Objectives
  • Initiate evaluation based on best practice evidence guidelines for acute pediatric conditions.
  • Coordinate care of pediatric patients with complex chronic conditions following national or best practice guidelines.
  • Assess and recognize pediatric patients who require emergent referral for acute surgical conditions.
  • Identify conditions that require referral to pediatric subspecialty care.
